The molar teeth in the upper jaw had been missing for a long time,

and the lower front teeth were loose due to periodontitis.

With the molars, which establish the occlusal height, missing,

the lower teeth had pushed up the upper teeth,

causing the upper front teeth to spread apart and leading to poor periodontal health.

Since the teeth had been missing for a long time,

the remaining bone thickness was too thin on both sides,

so we planned a surgery involving extensive bone grafting

and decided to proceed with navigation-guided implants,

then began the fabrication of the guide device.

Does surgery hurt a lot?

It doesn't hurt at all during surgery!!

This is a common question I receive from implant consultation patients.

Imagining implant surgery,

it might seem very painful because it involves placing implants into the bone,

but since the surrounding nerves are anesthetized with local anesthesia without general anesthesia,

there is no pain during the surgical procedure after anesthesia.

Before anesthesia, surface anesthesia is used to minimize discomfort,

and once anesthetized, you will not feel any pain during the surgery.

Bone was created up to the marked line,

and a much more sufficient bone height was secured than initially anticipated.

Sinus lift surgery is a procedure

that involves lifting the membrane of the maxillary sinus (=paranasal sinus)

and performing bone grafting in that space to create fixation for the implant.

If there had been sufficient existing bone, temporary teeth could have been made simultaneously with the surgery,

but since the initial fixation was weak,

I advised the patient to use a temporary denture for a while, despite the discomfort.

Treatment for extruded teeth and severe periodontitis

With the lower molars having extruded

and the front teeth loose due to severe periodontitis,

we treated the lower teeth while waiting for the implants.

First, we proceeded with gum treatment

to calm the gums affected by periodontitis,

extracted the problematic teeth, and used temporary teeth

to establish the occlusal height.

As gum health recovered through periodontal treatment,

and when it was time to use temporary implants,

the occlusal height was established,

and we could see that the previously splayed upper front teeth had settled into place.

Since the teeth had been missing for a long time,

a period was needed to achieve occlusal stability,

so we allowed a period for using temporary teeth

to check for temporomandibular joint stability, occlusal changes, and any discomfort during use,

and then determined the timing for the final prosthetics.

‘Eating is fine, and I have no discomfort~’

Despite using temporary teeth for quite a long period,

the patient didn't show any signs of difficulty and said that eating had become much more comfortable than before.

Here are photos after the occlusal stabilization period

and after the final prosthetics were completed.

Fortunately, the bone graft space was well maintained,

and about a year after starting treatment,

there were no signs of inflammation, and everything was well preserved.
