Shindang-dong Dental Clinic: Important Considerations for Anterior Prosthetic Treatment (Esthetic Prosthetics, Root Canal Treatment)

Learn about issues with old anterior dental prosthetics, including gum discomfort and inflammation, and how they are treated with root canal therapy and new esthetic crowns.

Shindang-dong Dental Clinic: Important Considerations for Anterior Prosthetic Treatment (Esthetic Prosthetics, Root Canal Treatment)

Hello.

I am Dr. Kim Tae-young, the head director of Bareunyoung Dental Clinic in Shindang-dong.^^

Prosthetic treatment that involves capping a tooth,

in other words, crown treatment, is performed when

a tooth is fractured or

a cavity develops, requiring restoration of the tooth's external shape.

Of course, the deeper the restoration area, the higher the possibility of needing root canal treatment.

Today, we will explore how problems can arise with previously crowned teeth

over time.

My gums around the old crowned tooth feel uncomfortable.

The patient visited us due to discomfort in the gums around an old anterior prosthetic.

The dental prosthetics were made at different times, with one or two teeth forming a single prosthetic unit.

First, if you look at the arrow, you'll see a round pus pocket on the gum.

It is believed that the boundary of the old prosthetic became contaminated

over time, leading to inflammation at the root of the tooth.

We took an X-ray for a closer look.

The X-ray clearly shows

that the inflammation inside has been present for several months.

In such cases, the old crown must be removed,

the inside thoroughly treated with root canal therapy, and then

a new esthetic prosthetic must be made to fit the tooth's shape.

At this time, if two teeth are bound together as one prosthetic unit,

they must inevitably be removed together, and then each tooth is fabricated separately.

Since the location of the inflammation was unusual, we took a CT scan.

Although the tooth is not mobile, the bone around the root appears to be significantly

lost due to the inflammatory reaction.

Generally, bone loss due to inflammation originating from the tooth nerve

will be refilled with healthy bone once healed after root canal treatment.

This was a case where pain occurred due to contamination at the boundary of the old prosthetic.

Following standard procedure,

we proceeded to the esthetic prosthetic stage after root canal treatment.

Shape and Color of the Final Prosthetic

This is a photo after the final anterior esthetic prosthetic was attached.

Just by replacing the new prosthetic,

the existing teeth also seem to have a softer appearance.

Considering the color of the surrounding teeth,

the old prosthetic had an unnatural boundary with the gum

and appeared bulky.

Taking that into account, we strived to create a natural tooth shape.

If the old prosthetic next to the newly treated tooth had also been replaced,

it would have looked much more natural,

but we decided to address the major discomfort first and consider it later.

The two problematic teeth received root canal treatment,

and the remaining one tooth was reinforced, completing the anterior prosthetic treatment without root canal therapy.

Going forward, we need to monitor whether the bone loss area due to inflammation

recovers.

Crown treatment can lead to

internal contamination of the prosthetic over time,

which is why the Academy of Prosthodontics recommends replacing prosthetics every 7-8 years.

Today, we looked at a case from Shindang-dong Dental Clinic

involving anterior tooth treatment and prosthetic replacement.

I hope this information was helpful to those who were curious about this topic.

Thank you.^^

Frequently Asked Questions

Why do old anterior dental prosthetics cause problems?

Yes, over time, the boundary of old prosthetics can become contaminated, leading to inflammation at the tooth root. This can cause gum discomfort or the formation of pus pockets.

How is inflammation under an old crown treated?

Typically, the existing crown is removed, and the inflamed area is thoroughly treated with root canal therapy. Afterward, a new esthetic prosthetic is fabricated to match the tooth's shape and then recapped.

Can only one tooth be replaced if prosthetics are bonded together?

No, if two or more teeth are bonded together as a single prosthetic unit, they must inevitably be removed together. Afterward, it is common practice to fabricate individual prosthetics for each tooth separately.

How often should dental prosthetics be replaced?

The Academy of Prosthodontics recommends replacing prosthetics every 7-8 years due to the possibility of internal contamination. Regular check-ups are crucial to monitor their condition.

What happens if bone is lost due to inflammation?

Generally, bone loss due to inflammation originating from the tooth nerve will be refilled with healthy bone once healed after root canal treatment. It is important to monitor the recovery process after treatment.
