The chief complaint was a desire to straighten the uneven upper front teeth.

Both canines appeared to require retreatment due to old prosthetics and restorations.

Among the remaining four front teeth, only the central teeth seemed slightly overlapped,

but the view from above was completely different.

Considering the degree of tooth protrusion, rotation to the left and right, and overlapping parts of the teeth,

even before creating a diagnostic model, it seemed that a significant amount of tooth reduction would be necessary to align the teeth.

I believe collaboration with a dental laboratory is crucial for aesthetic treatment of the front teeth.

First, to determine the amount of tooth reduction,

we request the dental laboratory to design the tooth shape.

The reason a diagnostic model was essential in this case is that the degree of tooth protrusion varied,

requiring a standard for tooth reduction.

Although veneer treatment for minimal tooth reduction was considered,

the diagnostic results indicated that crown treatment was more recommended from an aesthetic perspective.

Now, we need to trim the tooth's outer shape for the diagnostic form.

As mentioned earlier, veneer treatment, which involves reducing 0.x mm,

was not possible for this patient.

Some of the most prominent areas required more than 1mm of reduction.

Generally, reducing protruded teeth often leads to root canal treatment.

This patient's dentition also seemed like a case that would require simultaneous root canal treatment,

but an X-ray showed that the inside of the nerve canal was calcified and narrowed.

So, I thought it might be possible to proceed without root canal treatment.

Two of the six teeth had already undergone root canal treatment, and I wondered if we could preserve the nerves of the remaining four teeth

without root canal treatment.

Therefore, I explained to the patient that we would try to preserve the nerves and proceed with the treatment,

and if symptoms appeared later, we would perform root canal treatment.

In such cases, careful tooth reduction is required during the trimming stage,

and medication must be applied to prevent sensitivity during internal treatment,

and follow-up visits were needed to check for symptoms.

Nevertheless, the patient cooperated very well, and as a result, to date,

the four teeth were completed without root canal treatment and without any sensitivity.

This is the model with the final prosthetics fabricated and applied.

Although there were some difficulties during the temporary tooth fabrication process,

I promised the patient, who trusted and waited for me,

that the final prosthetics would be beautifully crafted.

When viewed from above, it forms a very ideal arch,

unlike before.

When viewed from the front, the midline is well aligned,

and the teeth are symmetrical

in shape. The patient wanted a natural color that wasn't too bright,

so it was completed in a bright shade within that range.

The six crowns were attached one by one to the teeth,

and then time was spent readjusting the occlusion.

Because the occlusion with the old teeth

and the occlusion with the new prosthetics are different,

contact must be formed in a way that does not interfere with tooth movement.

Adaptation process after final prosthetic attachment

Initially, adjustments are made in the dental office without occlusal interference,

but at first, the occlusion may feel awkward, and teeth may collide.

For all prosthetic treatments in the oral cavity, the final attachment is not the end;

it is essential to visit once or twice initially to check and adjust any occlusal interference

that arises during the adaptation process.
