Today, I'd like to talk about lower blepharoplasty.

Many people undergo various procedures and surgeries to improve their lower eyelids, but ultimately, they often decide on lower blepharoplasty, which involves skin incision.

Client desiring improvement of under-eye tear troughs (indented wrinkles)

Many people believe that if tear troughs are prominent as shown above, they can simply be filled with ‘fillers’ or ‘fat grafting’. However, the results are often unsatisfactory, and tear troughs cannot be fundamentally resolved with ‘fillers’ or ‘fat grafting’. This is because the wrinkles are caused by the tear trough ligament (TTL) pulling from the bone to the skin, so no matter how much you fill it, it cannot rise. The reason is that ligaments do not stretch easily. Overcorrection can even worsen the wrinkles.

As shown in the diagram above, since the ligament originating from the bone holds the skin, tear troughs cannot improve unless this ligament is cut or released.

Looking at the schematic diagram, even if ‘fillers’ or ‘fat grafting’ are used to fill the tear troughs, the ligaments do not stretch, so overcorrection can actually make the tear troughs appear deeper.

This client felt uncomfortable with surgical treatment and thought fillers were simple, so they received filler treatment, but their tear troughs actually worsened. They eventually revisited and underwent fat repositioning + septal reinforcement + midface lifting surgery, achieving very satisfactory results and regretting the filler treatment.

Looking at the surgical results, the aegyo-sal (under-eye fat pad) has become plump, the prominent malar bags below the aegyo-sal have receded, the tear troughs below the malar bags have become smooth, and the sunken midface has been lifted, resulting in lower eyelids that look much younger and more vibrant.

Ultimately, without surgically cutting the Tear Trough Ligament, deeply indented tear troughs are difficult to resolve fundamentally. Of course, for younger clients with less severe conditions, fillers or fat grafting may provide sufficient improvement.
