Droopy Eyelids: Before and After Incisional Double Eyelid Surgery for Ptosis
Explore the before and after of incisional double eyelid surgery for ptosis, addressing droopy eyelids that cause a sleepy appearance. This procedure improves both aesthetics and vision.

Double eyelid surgery is one of the plastic surgeries typically performed to improve both aesthetic and functional aspects.
Simply making the eyes larger does not solve all problems; it is crucial to consider the muscles and function to achieve not only cosmetic effects but also improved results in various aspects such as securing visual field and enhancing the power to open the eyes.
The causes of visual obstruction and eyelid drooping often stem from ptosis. Today, we will post about ptosis surgery, which goes beyond mere cosmetic issues and can also affect vision.

What is ptosis surgery?
Ptosis surgery is a procedure that corrects drooping eyelids to secure the visual field and improve appearance.
Ptosis can be congenital or acquired, often occurring as muscles weaken with age.
This can make the eyes look heavy and tired, or cause the eyelids to droop and obstruct vision.

Reasons why ptosis surgery is necessary
1. Securing visual field
The biggest reason is that drooping eyelids obstruct vision.
As the eyelids droop, it becomes difficult to fully open the eyes, which can lead to visual discomfort.
2. Aesthetic improvement
Ptosis can make the eyes appear small and constricted.
Improving eyelid drooping makes the eyes look clearer and more vibrant.
This is a natural phenomenon that occurs with age, and many people seek external changes through surgery.
3. Restoring confidence
Ptosis can make individuals self-conscious about their appearance, leading to a drop in confidence.
The improved appearance after surgery helps many people regain their self-confidence.

Problems: 1. Half of the pupil was covered due to eyelid drooping. 2. Heavy eyelids made the impression look stuffy and sleepy. 3. Discomfort when opening the eyes.

Surgical Plan: 1. Ptosis correction: Advancing the levator palpebrae superioris muscle and fixing it to the tarsal plate to improve ptosis. 2. Upper eyelid fat removal: Removing fat from the upper eyelids to alleviate the stuffy feeling. 3. Epicanthoplasty: Slightly opening the inner corner of the eye with epicanthoplasty to soften a strong impression. 4. Incisional double eyelid surgery: Minimally incising the eyelid, removing unnecessary tissue, and forming a double eyelid line.
This patient underwent their first eye surgery at our hospital, and due to severe ptosis, more than half of their pupil was covered.
They usually experienced discomfort when opening their eyes, leading to high eye fatigue, and had a stuffy impression.
