Bulbous Nose, Hump Nose, Long Nose Correction | Before & After Rhinoplasty Cases (NJZ Minji Nose)

This post details a rhinoplasty case for a 20-something female patient who desired a nose similar to NJZ Minji's. The procedure involved correcting a bulbous nose and a dorsal hump, using silicone for the bridge and septal and ear cartilage for the tip to achieve a semi-straight line.

The patient was a woman in her 20s who visited our clinic. While she didn't have a specific desired nose line, she had always found NJZ Minji's nose beautiful and wished to choose a line through consultation. NJZ Minji has a straight nose line with a rounded tip. A straight nose line is generally preferred by men, as its uncurved, upright straightness can convey masculinity. Therefore, proceeding with a personalized plan and design is crucial to achieving a satisfactory final result.

For women, we typically recommend a semi-straight bridge line to create a feminine or soft look. For the nose tip, we suggest a slightly rounded yet sharp appearance to achieve a natural feel without being too pointy, and this is also a shape preferred by patients. However, it's important to be careful, as an excessively rounded tip can have the opposite effect and create a masculine impression.

First Surgical Plan

This patient's surgical plan required correction of a slight dorsal hump and a bulbous nose. Silicone was used for the bridge, and cartilage was used to raise the nose tip. Since the patient's septal cartilage was sufficiently large and had good thickness, costal cartilage was not used. Instead, septal cartilage was used to create the basic support for the nose tip, and ear cartilage was used from the columella to the nose tip to establish the first plan for a semi-straight rhinoplasty.

During rhinoplasty, silicone is typically inserted to raise the nasal bridge, and cartilage is inserted into the nose tip. Cartilage can broadly be categorized into three types: septal cartilage, ear cartilage, and costal cartilage. The choice of material may vary depending on individual conditions or preferences, and we recommend making an accurate decision through consultation.

Before & After Comparison

When observing the length from the eyebrows to the nose tip, the patient had a relatively long midface. To make the nose appear shorter, when viewed from the side, the balance between the height of the glabella and the height of the nose tip should be such that the nose tip is slightly higher than the glabella. This angle makes the nasal angle more upright, which can make the nose appear shorter.

Therefore, the height of the silicone used for the nasal bridge, specifically the glabella height, was set at 2.5mm. For the nose tip, septal cartilage and ear cartilage were used to raise it by 4-5mm, and columellar lengthening was performed to make the columella parallel to the alar base and nostrils. The nose tip was shaped slightly sharper than NJZ Minji's, resulting in a more slender and sharper appearance of the nose tip when viewed from the front.

Precautions

First,

When making the nose tip sharp, it can easily appear pinched and unnatural. Therefore, to avoid an unnatural look, the nose should have a structure where the nasal line is thin but slightly widens at the end towards the alar base.

Second,

If the alar base widens and thins when raising the nasal bridge, the bulbous nose may not appear fully corrected. Therefore, by carving the sides of the silicone (a process of meticulously shaping the silicone to perfectly fit each individual's nose), the upper part of the silicone can be prevented from becoming too thick, allowing for a feminine and slender nasal bridge.

Third,

The ideal range for the columella, located between the lower border of the alar tissue and the nostril, is a parallel line.

Fourth,

If the nasolabial angle, where the lip meets the columella and nose tip, forms a straight line, it can create an unnatural appearance. To achieve a more natural result, the columella should be raised, and at the same time, the nose tip point should be designed slightly higher to give an upward-lifting impression.

Fifth,

If the silicone is not accurately carved, a silicone step or movement may occur. The height of the silicone envisioned during the nose surgery may not be correct, so it's possible to create a smooth nasal bridge by simultaneously inserting and carving during the procedure. It's also important to smoothly refine the soft tissues to prevent any step formation with the nose tip.
