Real Story 6: Male Eye, Nose, and Facial Contouring Transformation
A comprehensive transformation for a male patient in his 20s, addressing ptosis, a humped nose, and a weak chin to create a sharp, masculine appearance.

* Basic Information
# Male in his 20s
# Eyes
- Always uses the forehead muscles when opening eyes wide
- Eyes appear small and closed off at the outer corners, giving a stuffy and sharp/aggressive look
# Nose
- Low and short nasal bridge
- Bumpy bridge line due to a dorsal hump
# Face Shape
- Receding chin and rounded jawline
- Double chin and chubby cheeks creating a ‘turtle neck’ and blunt impression
* Diagnosis of Current Condition
# Eyes
- The patient wants to stop frowning and raising his eyebrows every time he opens his eyes.
- This habit is due to deep-set eyes combined with weak eye-opening muscles (ptosis).
- The outer corners are quite closed off, making the eyes look stifled. We planned to open them up refreshingly using VIBE’s signature ‘Mango Canthoplasty’.
# Nose
- The patient was highly stressed by the bumpy line caused by a low bridge combined with a hump.
- Due to a protruding mouth structure, the nose appeared relatively short.
- Fortunately, the septal cartilage had sufficient size and strength. However, since the skin was tight with little elasticity, we planned to raise the nose using only autologous cartilage without overextending.
# Face Shape
- The lower face is too short compared to the overall face length, lacking a masculine feel.
- The receding chin obscures the neck line, creating a blunt look. Even if double chin fat is reduced, the chin itself is short, so we planned to use an implant to define the chin tip.
- Buccal fat can be thought of as the ‘visceral fat’ of the face. It is the last to disappear no matter how much weight you lose. Simple surgical removal is the most convenient and efficient method.
* Surgical Plan
# Eyes
- Incisional Ptosis Correction
- Mango Canthoplasty (Lateral and Lower Canthoplasty)
# Nose
- Rhinoplasty (Hump reduction, silicone implant for the bridge)
- Tip Plasty (Extension graft using septal cartilage + Ear cartilage: Layered graft)
# Face Shape
- Buccal Fat Removal
- Ultrasonic Double Chin Liposuction
- Chin Implant

* Post-operative Progress
# Eyes
- Don’t the eyes look significantly larger and clearer compared to before?
- Compare the eyebrow positions before and after. Before surgery, he frowned and lifted his brows, creating forehead wrinkles. After surgery, you can see he opens his eyes well using only eye strength without excessive eyebrow movement.
- Since small eyes were a stress point, we planned the line to look like a thin ‘hidden’ double eyelid. While some men prefer a monolook, many look great with these thin inner-double eyelids.
- Even with Mango Canthoplasty, natural results are possible without scarring, excessive red mucosa exposure, or relapse.
# Nose
- Fortunately, the septum was straight and had enough strength, allowing for satisfying results with autologous cartilage. As the skin was tight, we focused on the line without overstraining the tissue.
- The nose is higher, making the facial profile look masculine and sophisticated. It also provided the ‘two-birds-with-one-stone’ effect of making the mouth appear less protruding.
# Face Shape
- How does the jawline look now that it’s defined? We achieved a sharp V-line from a U-shape, and the overall impression has become much more masculine!
- The bluntness disappeared with buccal fat removal and double chin liposuction. He went from a ‘rounded’ image to a sharp, sexy face shape.
- ‘LipoSound’ is a device that uses ultrasound to emulsify fat. It liquefies fat particles—like turning orange pulp into juice—allowing the fat to be suctioned out as smoothly as water without needing much force.

Therefore, there is much less irritation, leading to faster recovery from bruising and swelling. Post-operative adhesions, known as ‘bio-bonds,’ are also significantly reduced!
