Incisional Double Eyelid (4th Revision), Epicanthoplasty, and Under-eye Fat Grafting
A case study of a 20s female undergoing her 4th double eyelid revision to correct asymmetry, along with epicanthoplasty and under-eye fat grafting.

* Basic Information
# Female in her 20s
# Medical History: 3 previous revision surgeries due to line asymmetry, under-eye filler
# Current Status & Requirements: Despite multiple surgeries, the patient still experiences line asymmetry and wishes for correction. She also wants to improve her tired appearance caused by puffiness under the eyes.
* Consultation
# The patient was under significant stress due to multiple previous eye surgeries.
# She desired a clear, visible line starting from the inner corner.
# She felt the under-eye filler was excessive and wanted it removed, while also improving the puffy look under her eyes.
* Surgical Plan
# Incisional Double Eyelid (4th Revision)
# Epicanthoplasty (Right side only)
# Dissolving under-eye filler
# Under-eye fat grafting

* Post-operative Progress
# A light and fresh in-out line shape was achieved. Can you see how much clearer and more symmetrical the eyes have become?
# Through incisional revision, we removed all existing scar tissue and worked to create a natural look, as if it were her first surgery.
# The under-eye filler had spread downward, making the area look puffier. After dissolving the filler, it was clear that the actual amount of under-eye fat was not excessive. Therefore, we improved the tired look by performing fat grafting in the sunken tear trough area.
