Incisional Ptosis Correction + Epicanthoplasty
A woman in her 20s with asymmetric ptosis and 4mm skin sagging underwent incisional ptosis correction and epicanthoplasty, achieving natural, well-balanced results at 6 weeks post-op.

* Basic Information
Female in her 20s
Eye-opening strength: Weak on both sides, especially worse on the right (asymmetry)
Skin sagging: 4mm
Skin thickness: On the thinner side
The lacrimal lake appears blocked, giving a heavy look, and the pointed inner corners make the eyes appear sharp.
Request: Would like to resolve the ptosis asymmetry. Wants a natural-looking line!
* Consultation
1. The ptosis asymmetry is quite significant. Unless there is a specific memory of injury, this is most likely a congenital issue. Non-incisional ptosis correction has limitations when it comes to achieving bilateral symmetry, and given the severity of the ptosis, an incisional approach is recommended.
2. Epicanthoplasty is needed to address the asymmetry of the lacrimal lake and the sharp appearance of the inner corners.
3. Worried that an incision will leave visible scars or make the eyes look too intense? Absolutely not. Even with incisions, we can create a sufficiently natural, well-balanced line. There is nothing to worry about :)

* Post-Operative Progress
Since the patient had traveled from quite far away, we were unable to monitor her progress immediately after surgery — she came in for a follow-up 6 weeks later. Thankfully, she was extremely satisfied, and beautifully symmetrical eyes had been achieved. ^^ Moments like these are truly rewarding. Correcting asymmetric ptosis is, in fact, a considerably challenging procedure. Surgery must always be performed by an experienced board-certified plastic surgeon.
* Key Points
1. When is incisional ptosis correction indicated?
: When there is significant left-right ptosis asymmetry — fine adjustments for symmetry require incisional ptosis correction
: When the degree of ptosis is severe — non-incisional correction has its limits
: When skin sagging is present and appropriate skin excision is necessary
2. Will incisional ptosis correction make the eyes look harsh or give an outdated crease? I am also worried about scarring.
: If the line is designed to suit your specific eyes — for instance, the ideal crease height differs depending on whether you have prominent or deep-set eyes — they will absolutely not look harsh. And if the skin texture is carefully aligned during suturing, there is no need to worry about scarring!
Consultations and surgeries should always be performed by a board-certified plastic surgeon!
