In a case I introduced previously,

I mentioned that the nerve structure of our teeth is very complex and

has many variations, making the mechanical removal of nerves and

the cleaning process using medication extremely important.

Today, I would like to explain a structural variation of tooth nerves that appears with a very high incidence in the mandibular molars of East Asians.

The structure of the root canal, called a C-shaped canal, typically includes structures like C1, C2,

and various other variant structures as shown below.

When viewed in cross-section from above, as in photo C1,

the root canal entrances are all connected like the letter ‘C’,

making it more difficult to secure and disinfect the root canal

compared to teeth that generally have independent root canals.

Therefore, the recurrence rate after root canal treatment can be higher than in typical cases.

C-shaped canals mostly appear in the very last mandibular molars,

and because the roots of these teeth are fused,

they are structurally weak. When symptoms arise,

mobility and tooth cracks are often accompanied.

‘The filling I had before fell out.’

The restoration on the last lower left molar has fallen off.

The very end part of the tooth is chipped,

and an X-ray shows signs of inflammation at the root tip.

Although a crack has already progressed, the symptoms themselves were not severe, so we decided to perform root canal treatment and monitor the symptoms.

Indeed, the crack extends to the inside of the root.

Looking at the root canal structure of a similar tooth again,

the cross-section is connected,

and even within the actual tooth, there can be individual entrances,

but the root canals are connected like a valley,

so securing as many entrances as possible is paramount.

Fortunately, this is a photo after all root canals were secured and root canal treatment was completed.

Unlike other teeth, you can see that the root tips are fused.

Although temporary mobility is occurring due to the large extraction site of the wisdom tooth behind it,

I believe that if the pain during chewing is controlled, the current tooth can be preserved for a little longer.

When performing root canal treatment, even for teeth in the same area,

the location, variation, structure of the root canal, and root shape can differ,

so many factors are considered, and the approach changes with each treatment.

Even after prosthetic treatment,

I hope you can preserve your teeth for as long as possible

through adjustment and management.^^
