Baba Plastic Surgery: Incisional Double Eyelid Surgery, Incisional Ptosis Correction, Double Eyelid Revision Case Study

Today, we would like to introduce a case where our hospital's director personally underwent incisional double eyelid surgery at Baba.

Incisional Double Eyelid Revision Surgery

Surgical History

First surgery: 2nd year of middle school -> Surgery to make natural double eyelids more defined.

Second surgery: 5 years after the first surgery -> Revision surgery for loosened double eyelids + incisional ptosis correction.

Third surgery (current): Baba Plastic Surgery -> Incisional ptosis revision + double eyelid line incisional revision.

The notable point here is that despite having undergone double eyelid surgery twice, the ptosis had loosened again, giving the impression of drooping eyelids.

To identify the problem, we used AgnimA analysis to pinpoint issues and planned a suitable surgical approach.

MRD1: Distance between the pupil and the eyelid (checks the strength of eye opening).

Problems Identified

- Right MRD1: 3.0mm / Left MRD1: 3.6mm

(Ideally, 5-10% of the eyelid should cover the pupil, but the right eye was closed 1mm more than the left.)

- Eye size needed to be increased by more than 1mm.

(Non-incisional ptosis correction would not be sufficient to increase eye size.)

- Lowered double eyelid height.

- Insufficient eye-opening strength (loosened ptosis).

Desired Double Eyelid Line and Needs

- A line that considers the overall harmony and balance of the face.

- A naturally thick double eyelid line, not a ‘sausage line’ caused by over-correction.

- A double eyelid line where the inner corner of the eye does not appear lifted.

Surgical Plan

- Incisional double eyelid surgery + incisional ptosis correction + lateral canthoplasty.

- Left double eyelid line raised by 2-2.5mm for a glamorous in-out line.

(A glamorous in-out line with a normal inter-eye distance and high line height: AIHT)

- Right pupil -> Ptosis correction to match the left pupil for a clearer look + line symmetry adjustment.

- Lateral canthoplasty (performed to the maximum extent possible through AgnimA eye analysis, avoiding over-correction).

The incisional double eyelid surgery method involves minimal incision to remove unnecessary muscle, minimizing scarring and promoting faster recovery. With delicate suturing techniques, the surgery can achieve natural results where scars are not visible even when the eyes are closed. For lateral canthoplasty, the skin blocking the outer corner of the eye is micro-incised by about 4-5mm, aligning with the angle of the outer corner, and then delicately sutured to prevent reattachment.

*When canthoplasty is performed simultaneously, conjunctival edema may occur, but this often subsides naturally over time. However, if conjunctival edema does not disappear after a certain period, it is necessary to visit the hospital and receive treatment according to the specialist's instructions.

Surgical Results

- Right MRD1: 4.1mm / Left MRD1: 4.2mm

Through incisional double eyelid surgery, incisional ptosis correction, and lateral canthoplasty, we were able to achieve a clear and ideal double eyelid line with balanced thickness and symmetry.
