This surgery combined:

1️⃣ Functional Rhinoplasty (Septoplasty)

Straightened the deviated septum to improve airflow.

Reduced the hypertrophied inferior turbinate to alleviate nasal congestion.

2️⃣ Bulbous Nose Correction

A bulbous nose occurs when the tip cartilage (lower lateral cartilage) is spread apart, the cartilage is thick, or the subcutaneous fat is thick.

Therefore, it is not just about reducing the alar wings; the shape and angle of the cartilage must be meticulously adjusted.

In this case:

Used the septal cartilage as a central pillar (strut) to secure tip support.

Added ear cartilage grafts to complete a natural-looking tip.

Used a thin silicone implant for the bridge to create a sophisticated ‘straight-semi-basal’ line.

Through this process, the tip was firmly yet naturally gathered, and a harmonious line flowing smoothly from the bridge to the tip was completed.

After the surgery:

The spread-out bulbous nose was refined into a three-dimensional tip.

The bridge was naturally connected, making the facial midline clearer.
