Deviated Nose and Drooping Tip (Revision Surgery)
A case study of a male patient in his 30s who underwent revision rhinoplasty to correct a deviated nose and drooping tip caused by a weakened septal support.

* Basic Information
# Male in his 30s
# Previously underwent rhinoplasty using septal cartilage, but the nose appears deviated to one side. Over time, the nasal tip has dropped, making the bridge look bulging.
* Consultation
# The patient visited due to two main issues following a previous septal rhinoplasty: 1. A nasal tip that became increasingly deviated over time, and 2. A drooping nasal tip that caused the bridge to appear humped or bulging.
# The septum itself was deviated, and the accumulation of postoperative scar tissue was making the deviation appear even more pronounced.

* Intraoperative Findings
# Severe scar tissue was present; it was thoroughly removed, and the area was irrigated with an anti-scarring agent.
# The septum was deviated, and the previous septal extension graft was weakly fixed to it. This was the primary cause of the deviated nasal tip.
# The septum was reconstructed. A sturdy new septal pillar was created using the existing septal cartilage combined with new donated rib cartilage.
# Additional cartilage was grafted onto this strong pillar to increase the height of the nasal tip.
# When performing revision surgery, it is crucial to establish a perfect plan and prepare thoroughly beforehand. One must consider various variables in advance to respond flexibly to unexpected situations that may arise during surgery!
