* Basic Information

# Female in her 30s

# Several years ago, she underwent rhinoplasty using septal cartilage, donated rib cartilage, and a silicone implant. However, she later had the silicone implant removed because it looked too high. Since then, she has wanted to improve the flat appearance of her nasal bridge, while also addressing her crooked nose and breathing difficulties.

* Consultation

# The patient wished to improve the flat appearance of her nasal bridge that resulted from the removal of the silicone implant.

# Although she had tip surgery using septal and donated rib cartilage, she was dissatisfied as the columella gradually became crooked over time, leading to a blocked nostril and breathing discomfort.

# Pre-operative CT scans showed severe deviation, so a plan was made to perform the correction using autologous rib cartilage.

* Intraoperative Findings

# Severe scar tissue was present, which was cleanly removed, and the area was irrigated using an anti-scarring agent.

# The septum was severely deviated, and the septal extension graft was weakly fixed to that area. This was the cause of the crooked nasal tip.

# The septum was newly reconstructed using autologous rib cartilage. By improving the deviated septum, the nasal congestion symptoms were also corrected.

* Postoperative Findings

# Autologous rib cartilage is typically harvested from the 7th or 8th rib. There is minor pain for about 2-3 days after harvesting, but recovery is sufficient to return to daily life after about one week.

# When performing revision surgery, it is crucial to plan and prepare perfectly beforehand. One must consider various variables in advance to respond flexibly to unexpected situations that may arise during surgery!
