Deviated Nose Revision Surgery! (+ Fat Grafting)
Case study of a 20s female patient undergoing revision rhinoplasty for a deviated nose and high bridge, combined with forehead fat grafting for a feminine profile.
* Basic Information
# Female in her 20s
# Medical History: Two previous rhinoplasty procedures using septal cartilage and donated costal cartilage.
# Current Condition: The bridge is excessively high and the tip of the nose is severely deviated.
* Consultation
# The primary issues after two surgeries were a long, drooping columella and a central axis of the nasal tip that was severely deviated to the left. We planned to create a straight line by correcting the deviated nose.
# Despite a lack of volume in the forehead, the nasal bridge was too high, creating an ‘Avatar-like’ appearance. We planned to lower the bridge while adding volume to the forehead to create a more feminine look.
# Since a long nasal tip can make one look older than their actual age, we planned to create an appropriate tip shape and angle.

1. The 7th rib cartilage on the right side was harvested and used. Since autologous rib cartilage is harvested through a short incision of about 3cm, you do not need to worry much about scarring. In cases of severe deviation like this one, it is crucial to use strong cartilage, such as autologous rib cartilage, to ensure definitive correction.
2. Deviated nose surgery is a high-difficulty procedure, but patient satisfaction tends to be very high after surgery. This is because it resolves the crooked appearance from the front and straightens the central axis.
3. A satisfying result was achieved by correcting the long, drooping columella and creating an ideal nasal tip shape and angle.
4. When the forehead lacks volume, the face can appear flatter and longer. Combining the procedure with fat grafting can lead to even more satisfying results.
