# Female in her 30s

# Underwent rhinoplasty several years ago using AlloDerm and donated rib cartilage, but was dissatisfied with the shape.

* Consultation

# Although she had surgery using artificial dermis (AlloDerm) for the bridge and donated rib cartilage for the tip, she was unhappy with the aesthetic outcome. Over time, the nasal tip appeared increasingly deviated, and nostril asymmetry developed, which were her primary concerns.

# The plan was to correct the deviated nose using new donated rib cartilage and to raise the nasal bridge using an implant.

* Intraoperative Findings

# Severe scar tissue was present; it was cleanly removed, and the area was irrigated with an anti-scarring agent.

# The nasal septum was severely deviated, and the previous septal extension graft was weakly fixed to that area. This was the cause of the deviated nasal tip.

# The nasal septum was reconstructed. A sturdy septal pillar was newly created using new donated rib cartilage.

* Postoperative Findings

# You can see that the deviated nose has been corrected and the nostril asymmetry has been sufficiently resolved.

# With the bridge properly defined, a sharp and sophisticated nasal profile was achieved.

# When performing revision surgery, it is crucial to establish a perfect plan and prepare thoroughly beforehand. One must consider various variables in advance to respond flexibly to unexpected situations that may arise during surgery!
