Correction of Short Bulbous Nose and Pinched Nostrils (Alar Rim Lowering)
A case study of a 30-year-old female undergoing revision rhinoplasty to correct a short, bulbous nose and pinched nostrils caused by a previous L-shaped implant.

* Basic Information
# Female in her 30s
# Medical History: Previous L-shaped implant surgery (+)
# Patient Complaint: ‘As time passes, the tip of my nose feels like it is getting blunter and shorter. I particularly want to correct my right nostril because it looks pinched.’
* Consultation
# Although she only had L-shaped implant surgery in the past, she wanted improvement as the tip of her nose appeared more upturned over time. She also requested to refine the blunt nasal tip and specifically address the pinched appearance of the right nostril.
# The appearance of a pinched nostril is called ‘alar retraction.’ This requires correction because it gives an artificial, operated look and an unnatural impression.
# We expect a satisfying nasal line by refining the nasal tip while simultaneously correcting the short and pinched nose.
* Surgical Plan
# Tip Plasty (Bulbous, Short, and Pinched Nose Correction)
- Septal Extension Graft
- Alar Rim Lowering (Nostril Lowering)
# Rhinoplasty (Implant Replacement)

* Post-operative Progress
# You can see that the blunt nasal tip has been refined, and the pinched right nostril has significantly improved.
# If an implant is placed too deep, or if an L-shaped implant is used as in this case, contracture due to scar tissue can occur, which may lead to a pinched nostril appearance.
# Excessive nostril exposure makes the nose look shorter and can create an unfavorable impression. It is essential to reduce the visible size of the nostrils through accurate cause analysis.
What if your nostrils are too visible?? (1) If the alar rim is retracted -> Alar rim lowering (2) If the columella is drooping -> Columella correction (3) If the alar base is wide -> Alar reduction
