Today I'd like to review a natural adhesion (dermal adhesion) case.

Before Surgery

The patient already had a double eyelid line, but it was not holding stably and had loosened.

In addition, due to facial asymmetry, both eyes showed differently shaped contours.

The right eye had a longer horizontal length and a relatively flat shape,

while the left eye had a greater vertical length but a shorter horizontal width, giving it a more rounded appearance.

Diagnosis & Surgical Plan

To more aggressively correct the bilateral eye asymmetry,

more precise symmetry correction is possible through methods such as incisional double eyelid surgery,

ptosis correction, and excision of sagging skin.

However, the patient did not want correction to that degree

and preferred a direction that naturally refined the line while easing the asymmetry.

Accordingly, the following surgical plan was established.

✅Non-incisional natural adhesion double eyelid surgery

✅No ptosis correction performed (considering the limits of correction with the non-incisional method)

✅Approach the asymmetry by refining the line

The design the patient wanted was a semi-outline,

aiming for a line that does not tuck inside the epicanthal fold

but flows naturally outward.

2 Weeks Post-Op

In the early stage, swelling can make the double eyelid appear somewhat deep and unnatural,

but this is a normal part of the recovery process.

✅The line has stabilized in a semi-outline shape

✅The height of both double eyelids is set similarly

✅Some subtle asymmetry remains due to inherent differences in eye size

3 Months Post-Op

In the 3-month follow-up photos, you can see that the swelling has subsided sufficiently

and the line has settled into a much softer shape than in the early stage.

The semi-outline the patient wanted does not tuck inside the epicanthal fold at the front;

it flows naturally outward and is clearly defined,

and the height of both double eyelids is also well balanced,

showing that the asymmetry has been noticeably eased compared to before surgery.

Some structural differences from the horizontal-to-vertical ratio of the eyes remain,

but with line correction alone, the overall impression

has been refined into a more polished eye shape.

✅As swelling subsides, the line settles more naturally

✅Both eyes are more harmonious and the asymmetry is eased

✅A soft, natural-looking double eyelid is complete overall
