* Basic Information

# Female in her 30s

# Approximately 1 year ago, she underwent a ‘diced cartilage graft’ at another clinic.

# Since then, the nasal bridge has become bumpy and shows visible boundaries.

* Consultation

1. This patient had previously consulted with me for a no-implant rhinoplasty. However, due to a fear of surgery, she hesitated and eventually chose another clinic after being told, ‘All you need is a graft using your own diced cartilage.’ Unfortunately, she returned to us after developing issues such as an uneven nasal bridge and cartilage becoming visible and palpable.

2. Since a significant amount of cartilage had been used in the previous surgery and she desired a more glamorous look for her revision, we planned to proceed using autologous rib cartilage.

* Surgical Plan

1. Nasal Bridge Rhinoplasty

- Removal of foreign materials (previous grafts)

- Correction of the remaining hump

2. Nasal Tip Rhinoplasty

- Use of autologous rib cartilage

- Removal of visible cartilage at the tip and reinforcement of skin thickness

- Correction of the flat columella to a more natural, rounded shape

- Changing the current overly straight profile to a ‘straight-semi-basal’ (S-line) profile

* Intraoperative Findings and Postoperative Progress

# Diced cartilage fragments were scattered and grafted everywhere under the skin. It took a considerable amount of time to remove them all.

# The biggest challenge in these cases is that the grafts are tightly adhered to the underside of the skin. Removing them can make the skin extremely thin, and there is even a risk of skin perforation if not careful. It is crucial to use dermis grafts or similar methods to address the thinned skin and ensure the surface appears as smooth as possible.

# I did my absolute best to eliminate the bumpiness, and fortunately, the result was very satisfying. Both the patient and I were very pleased with the outcome!
