We introduce a double eyelid line lowering surgery for cases where the eyelid crease appears too high and gives a "sausage-like" appearance. A new line is created below the existing incision, and if the skin (scar tissue) between them is excised, it's called the "one-line method"; if left unexcised, it's called the "two-line method." This case demonstrates natural results maintained at 2 months, 4 months, 6 months, and over 1 year after lowering the line and partially excising the skin.

Why Lower the Double Eyelid Line?

First, when you want to reduce the line height.

Second, because of the "sausage-like" appearance. The higher the line, the more sausage-like the eyes appear.

When lowering the double eyelid line, a new line is created below the original incision. Then you decide whether to excise the scar tissue between them or leave it as is.

If you leave it as is, it's called the "two-line method," and if you excise the scar tissue between them, it's called the "one-line method."

Of course, "two-line method" and "one-line method" are colloquial terms, NOT official medical terminology.

In the diagram above, if the brown line is the original incision, the blue line represents the new incision below it. After that, you must decide whether to excise the skin between them.

This patient wanted surgery because the double eyelid line appeared higher and more unnatural than desired.

Since it looked like a "sausage eye," the line was lowered and some skin was excised.

This is the appearance after suture removal. A small portion of scar tissue at the line was excised, and the fixation was adjusted to a lighter tension.

However, scarring does not completely disappear. Scar formation depends on the extent of underlying tissue damage.

This is the appearance at 2 months post-op. There is still some swelling present.

The patient visited because it appeared there might be inflammation.

At 4 months post-op, the line has lowered significantly and the sausage-like appearance has improved.

This is the appearance at 6 months post-op.

The patient mentioned that the right eye in the photo (left eye from the patient's perspective) appears slightly fuller, but in reality it is just a subtle difference within the natural range with no significant asymmetry.

During follow-up, the patient noticed a black dot-like spot on the left eyelid line and found that a remaining suture was present.

Fortunately, we rechecked it at the clinic. A pimple-like lesion developed at the site where the remaining suture had been, rather than actual inflammation. See photo above.

Now, over a year post-op, the shape appears to be well maintained.

The overall impression and eye appearance are so well-refined that this case deserves to be featured as a representative clinic example. I'm posting it for reference. 😊
