Primary Rhinoplasty Case: Woman in Her 20s

Thin implant for the bridge + Hump correction · Osteotomy, Bulbous tip · Nasolabial angle correction for a natural result

Hello.

I am Dr. Young-moon Yoo, Director of Vibe Plastic Surgery.

Today, I would like to introduce a primary rhinoplasty case for a woman in her 20s.

This is a helpful case for those considering their first nose surgery to understand how we establish a surgical plan.

Reasons for Considering Primary Rhinoplasty

This patient had been concerned for a long time because her nose appeared large relative to her face, and especially from the front, it gave a blunt and dull impression.

✔️ The nasal bridge was not particularly low,

✔️ But there was a slight hump protruding,

✔️ The tip appeared dull and spread out,

✔️ And a low nasolabial angle made the mouth look slightly protruded.

These combined factors made her face look relatively closed-off and heavy.

The Most Important Point for Primary Rhinoplasty in Your 20s

The most important thing in primary rhinoplasty is not ‘heightening at all costs,’ but rather:

👉 Size and proportions that fit the face

👉 Natural changes

👉 A design that minimizes the possibility of revision surgery.

Therefore, for this patient, the goal was to create a refined and sophisticated image rather than an excessive transformation.

Surgical Plan ① Nasal Bridge – Ultra-thin Implant + Hump Correction + Osteotomy

✔️ Why an ‘ultra-thin’ bridge implant?

Since the patient’s existing bridge was not very low, a thick implant could have created an artificial look.

So, we planned to 👉 use only an ultra-thin implant just to refine the line.

Natural bridge

Smooth side profile

Minimal signs of surgery

✔️ The Role of Hump Correction and Osteotomy

The hump in the middle of the bridge is one of the reasons the line can look rough.

Through hump correction, we smoothed out the uneven areas and performed an osteotomy to narrow the width of the nose, adjusting the overall size to fit her face.

→ This achieves a slimmer and softer appearance when viewed from the front.

Surgical Plan ② Nasal Tip – Bulbous Nose and Nasolabial Angle Correction

✔️ Bulbous Nose Correction

This patient had relatively thick soft tissue at the tip, a typical bulbous nose type where the tip looks spread out from the front.

Rather than just suturing, we reorganized the tip structure and established support to

👉 focus on reducing the dull feeling and creating a defined tip.

✔️ Nasolabial Angle Correction

If the nasolabial angle is low, the mouth can look protruded, or the face can look heavy even without expression.

We designed the nasolabial angle correction to be subtle, making the side profile smoother and the overall facial proportions look more stable.

Post-Surgery Changes

After the surgery:

The overall size of the nose was reduced to fit the face.

The hump was smoothed out, resulting in a soft bridge line.

The tip, which looked blunt from the front, was refined to be slim.

The impression around the mouth became much more relaxed due to the improved nasolabial angle.

This resulted in a natural change that was not excessive, even for a first surgery.

Recommended for Those Who:

Are considering their first rhinoplasty in their 20s

Feel their nose is large and blunt compared to their face

Want to correct both a hump and a bulbous tip

Do not necessarily want a high bridge but want to refine the line

Want natural and long-lasting results

For Primary Rhinoplasty, ‘Moderation’ is Key

Since the first nose surgery is the one that changes the facial image for the first time, one must be even more cautious.

At Vibe Plastic Surgery, we design the most natural direction for primary rhinoplasty after thoroughly analyzing the patient’s facial proportions and existing nasal condition.

We always aim for honest and stable surgery so that the first surgery can be the most satisfying final surgery.
