Revision eye surgery requires a high level of expertise and delicate technique because ‘tissue damage is higher and the patient’s psychological burden is greater’ compared to the initial surgery.

Today, I would like to introduce a revision case of a patient who traveled all the way from Japan to Vibe Plastic Surgery for her third eye surgery. Interestingly, she was a former patient of mine who had previously received rhinoplasty from me. Although she had suffered emotionally due to frequent surgeries and unnatural results, both the patient and I were extremely satisfied with the outcome of this procedure.

⚠️ Patient’s Condition and Concerns Before Surgery

The patient had already undergone two previous eye surgeries at other clinics.

1st Surgery: Natural Adhesion (Non-incisional)

2nd Surgery: Incisional Ptosis Correction

However, even after the second surgery (incisional ptosis correction), she suffered from significant stress due to the following issues:

• Sausage Eyes (Thick Double Eyelids): The lines were set too high, creating an unnatural and heavy impression.
• Line Asymmetry (Uneven Eyes): The height and shape of the double eyelid lines on both sides did not match.
• Sunken Eyelids: A lack of volume in the upper eyelids made the eyes look hollow and tired.

💡 Vibe’s Customized Revision Solution

After accurately diagnosing the patient’s eye condition, I established a complex revision plan to delicately release existing scar tissue, naturally lower the lines, and restore volume.

1. Incisional Ptosis Correction Revision (Lowering Thick Lines & Correcting Asymmetry): I released the existing high and deeply adhered lines and designed new lines at a natural height that suited the proportions of the patient’s facial features. Simultaneously, I precisely adjusted the eyelid opening strength and symmetry on both sides to resolve the stress of uneven eyes.
2. Sunken Eyelid Fat Repositioning & Fat Grafting (Resolving Hollow Eyelids): When the upper eyelids are sunken, the eyes look more tired, and multiple folds can form. I repositioned the remaining fat appropriately and used micro-fat grafting for the deficient areas to fill in natural volume, completing a more vibrant and smooth eye shape.

✨ Surgical Results

The result was a great success. The ‘thick sausage lines’ and ‘asymmetry’ that had troubled the patient were cleanly improved, resulting in very natural and clear eyes even without makeup. Furthermore, through the eyelid fat grafting, volume was restored to the sunken eyelids, changing her look to a much younger and softer impression.

The patient, who had many worries since it was her third surgery, was overjoyed and highly satisfied with the results. As the surgeon, I also felt a great sense of accomplishment because the outcome was so positive.
