Bulbous Nose Rhinoplasty and Columella Correction
A case study of a teenage female patient who underwent bulbous nose correction and columella lengthening to achieve a more refined, sophisticated facial profile.

* Basic Information
# Female in her teens
# The nose is low and bulbous. A retracted columella makes the nose appear even more blunt.
* Consultation
# The patient visited to correct a bulbous nose. While traditionally called a ‘lucky nose’ in Korea, many people seek improvement because it can look heavy or blunt. Additionally, the significantly retracted columella gives a somewhat ‘stifled’ impression. We decided to perform columella lengthening (columellar extension) to create an ideal nasolabial angle.
# By refining the nasal tip while raising and smoothing the bridge, we can achieve a satisfying nasal line. In particular, the upper part of the nasal tip is bulging, so smoothing this area will be a crucial part of the process.
# Since the forehead lacks volume, I also recommended fat grafting, but she decided to save that for another time.
* Surgical Plan
# Nasal Tip Surgery (Bulbous nose correction, Columella lengthening)
# Nasal Bridge Surgery (Osteotomy, Silicone implant)

* Post-operative Progress
# While correcting the bulbous nose, we simultaneously extended the retracted columella to create an ideal nasolabial angle.
# The patient was very satisfied as the surgery resulted in a much more sophisticated look.
# If she undergoes forehead fat grafting in the future, her profile line will be even more enhanced.
