​# The double chin area is composed of various layers as shown in the following diagram. Let’s look at each component in more detail:

A. Fat

1. The procedure commonly known as LipoSound liposuction targets the subcutaneous fat layer.

​2. However, as you can see in the diagram, fat also exists beneath the platysma muscle (subplatysma fat, interplatysma fat, etc.). Since this is deep fat located under the muscle, it can never be reduced through suction alone. Therefore, cases where the area still looks bulgy or blunt after liposuction are often due to this type of fat. This fat must be removed by making an incision in the muscle and directly visualizing it!

B. Muscle

In some cases, the platysma muscle itself may be sagging. This can be congenital or a result of the natural aging process. In such cases, the muscle must be tightened firmly, much like wearing a ‘corset’, to create a tight line. This procedure is commonly called ‘double chin muscle tying’. Since this method has long been used in neck lifts, it is a proven and safe technique.

1. It is much smoother—fat can be squeezed out like water without applying much force, and it can be suctioned using only a small cannula. This results in less irritation, leading to faster recovery from bruising and swelling, and significantly less ‘bio-bond’ (post-operative adhesions).
2. Since ultrasound is applied beneath the dermis, there is less skin sagging after the procedure, and additional skin elasticity and tightening effects can be achieved.

# Bio-bonds may form starting about two weeks after surgery, which can be uncomfortable. In such cases, applying warm compresses and stretching will help them resolve faster. We also provide injections to help dissolve them during the one-month follow-up, so please keep that in mind!
