Today, I would like to introduce a case of revision rhinoplasty for a male patient in his 20s.

The patient had previously undergone rhinoplasty at another clinic but visited us due to dissatisfaction with the results.

👨‍💼 Patient’s Primary Concerns

This patient wanted to improve two main aspects:

1️⃣ The bridge of the nose had a curved, feminine ‘half-curved’ (ban-beoseon) shape after the initial surgery.

2️⃣ The nostrils were excessively exposed from the front, creating a ‘dragon nose’ appearance.

For men, there is a strong preference for a straight nasal bridge that creates a sharp impression and a stable tip where the nostrils are less visible.

This surgery focused on restoring this masculine balance.

✨ 1. Additional Dermis Tissue (Megaderm) Graft Under the Implant

Instead of replacing the existing implant, we performed an additional graft of dermis tissue (Megaderm) underneath the implant.

The reason for choosing this method was:

Since the patient was satisfied with the width and feel of the existing implant, we utilized it to avoid an overly drastic change in impression. This allowed us to make the nasal line more natural and smooth, effectively transforming the feminine curved line into a masculine straight line.

👃 2. ‘Alar Rim Lowering’ Using Ear Cartilage Composite Graft

When nostrils are highly visible from the front (often called a ‘dragon nose’), ‘alar rim lowering’ using an ear cartilage composite graft is effective.

This procedure naturally lowers the alar rim, making the nostrils less prominent from the front.

In other words, it achieves two effects simultaneously: correcting nostril exposure and softening the overall impression.

👉 This method is highly recommended for those concerned with nostril or alar symmetry and exposure.

🔹 3. Tip Reinforcement and Long Nose Correction with Ear Cartilage Graft

Additionally, ear cartilage was grafted to the tip to increase height and delicately adjust the angle of the nasal tip.

As a result:

The structural stability of the tip was maintained,

And the ‘long nose’ appearance that looked droopy was corrected into a shorter, more sophisticated shape.

A natural and well-balanced straight nose was completed, making the overall facial proportions much more harmonious.

📸 Post-Operative Changes

The feminine, curved feel disappeared,

A masculine straight nasal line was achieved,

And the nostril exposure was reduced, resulting in a balanced facial impression.

Most importantly, the result was a natural and neat look without any artificial feel.
