Today, I would like to introduce a revision surgery case of a female patient in her 60s whose main concerns were:
👉 Lower eyelids sagging and drooping again over time despite having lower blepharoplasty about 10 years ago.
👉 A strong and heavy impression caused by sagging eyebrows and upper eyelids.
📌 Pre-operative Status Summary
This patient was experiencing complex aging around the entire eye area, not just the lower eyelids.
- History of lower blepharoplasty about 10 years ago
- Dark and tired impression due to recurring sagging of lower eyelid skin and fat
- A heavy look caused by a very narrow gap between the eyebrows and eyes
- Heavy and sleepy impression due to upper eyelid sagging
👉 In such cases, there are limits to what a single procedure can achieve. It is crucial to have a surgical plan that identifies the causes and addresses them step-by-step.
1️⃣ Endoscopic Brow Lift
A procedure particularly effective for upper eyelid sagging caused by brow ptosis.
When the upper eyelids sag, many people think:
👉 ‘Shouldn’t I just get double eyelid surgery or an upper blepharoplasty?’
However, in cases like this patient where the eyebrows themselves have descended, operating only on the upper eyelids will not sufficiently resolve the heavy, stuffy look.
✔ What is an Endoscopic Brow Lift?
An endoscopic brow lift is a procedure that lifts the sagging forehead and eyebrows through small incisions (1–1.2 cm) inside the scalp.
The key is to restore the eyebrows to their normal position while minimizing the burden of visible scarring.
✔ Why it was particularly effective for this patient:
The eyebrows had sagged downward → narrowing the space between the eyes and brows, making the upper eyelids appear even more saggy.
Through the endoscopic brow lift:
- The brow position was naturally restored upward
- Space was secured between the eyes and eyebrows
- Upper eyelid sagging improved significantly and felt ‘lighter’ without any excision
👉 The forced look of straining to open the eyes disappeared, changing into a comfortable and refreshed eye shape.

Satisfaction is particularly high in the following cases:

2️⃣ Lower Blepharoplasty Revision + Mid-face Lift
The fundamental way to resolve recurring lower eyelid sagging.
✔ Why Lower Blepharoplasty Revision is Difficult:
Revision surgery for the lower eyelids requires a much higher level of skill than the initial surgery. This is because of:
- Tissue adhesions from the previous surgery
- Structural changes in the skin, muscle, and fat layers
- Potential lack of skin elasticity due to excessive excision in the past
👉 If approached incorrectly, it can lead to the eyelid turning outward (ectropion) or an unnatural look, so experience and accurate diagnosis are more important than anything.
✔ The Most Important Point in Revision Surgery:
For this patient, we did not simply ‘cut away the sagging skin again.’
👉 We approached it by restoring the structure that supports the lower eyelid itself.
Therefore, we chose a combination of lower blepharoplasty revision and a mid-face lift.
✔ Effects of Combining a Mid-face Lift:
A mid-face lift pulls up the sagging soft tissue between the lower eyelid and the cheekbone, acting as a support for the lower eyelid from below.
Because of this:
- The possibility of recurring lower eyelid sagging is reduced
- Dark and hollow-looking lower eyelids are improved
- The area above the nasolabial folds is naturally lifted
- The entire face changes to a softer, more youthful impression
👉 One can expect more stable and long-lasting results compared to a simple lower blepharoplasty.

- Brow position
- Upper eyelid sagging
