Rhinoplasty for a Low Bulbous Nose
A case study of a 20-year-old female patient who achieved a refined, sophisticated look through bulbous nose correction and bridge augmentation using septal extension grafts.

# Low and blunt nose
* Consultation
# The patient desired improvement because the bridge was low and the tip of the nose appeared blunt (bulbous).
# Preoperative CT scans showed that the septal cartilage was straight and sufficient in size. Therefore, we decided to raise the nasal tip using a septal extension graft and augment the bridge using a silicone implant.
# Since raising the nasal tip alone would sufficiently improve the appearance of the wide alar (nostrils), we decided not to perform a separate alar reduction.

* Postoperative Progress
# There are several types of bulbous noses: 1. Thick skin, 2. Thick soft tissue (subcutaneous fat, etc.), 3. Spread and wide cartilage. Since these factors usually occur in combination, it is best to correct each factor appropriately. This involves sculpting the skin and soft tissue to make them thinner, and reshaping the cartilage to create a sleek, defined look.
# We achieved the ideal nasal tip angle using septal cartilage and ear cartilage.
# Now that the bulbous nose has been corrected, the patient’s overall impression has changed to a more sophisticated one!
