The firm part felt in the lower eyelid is called the tarsal plate. Lower canthoplasty is a procedure where this tarsal plate is fixed to the lower septum or periosteum to pull it downward. As a result, skin and tissue naturally bunch up in the under-eye area. Therefore, it is difficult to call this a side effect; rather, it is a phenomenon where tissue bunching accompanies the lowering effect of the surgery.
For patients requiring a significant amount of lowering or those who naturally have thick under-eye tissue, I sometimes include a process to excise the rolling tissue through a small incision in the skin to prevent this phenomenon. Since the incision is made just below the eyelashes, there is no need to worry about scarring, and it allows for correction so the under-eye area does not look overly bunched, leading to high patient satisfaction.
