The area around the eyes is one of the first places to show signs of aging.
By the time people reach their 60s, they often experience:
Severe drooping of the eyelids,
Deepening forehead wrinkles due to using forehead muscles to open the eyes,
And sagging under the eyes.
The case I am introducing today is a 60-year-old mother who visited our clinic with these exact concerns.
* Pre-operative Concerns
This patient presented with the following characteristics:
✔ Severe skin sagging on the upper eyelids
✔ Deep forehead wrinkles from excessive use of forehead muscles when opening eyes
✔ Sagging skin and protruding fat under the eyes
✔ An overall tired-looking impression
In such cases, a simple double eyelid surgery is often insufficient for significant improvement.
This is because sagging around the eyes is a structural issue where the forehead, eyelids, under-eye area, and midface are all interconnected.
Therefore, we combined the following procedures for this surgery:
Endoscopic Forehead Lift
Sub-brow Lift
Natural Adhesion Blepharoplasty
Lower Blepharoplasty + Midface Lift
The goal was to achieve natural improvement by harmoniously combining each procedure.
1️⃣ Endoscopic Forehead Lift – Fundamental Solution for Forehead Wrinkles and Eye Sagging
For many with severely drooping eyelids, the ‘eyebrows themselves have often descended’.
If only double eyelid surgery is performed in these cases:
The eyes may look unnaturally large,
The double eyelid fold may become too thick,
Or the overall impression may become too harsh.
Therefore, in this case, we used an Endoscopic Forehead Lift to:

In patients over 60, the eyelid skin itself is often significantly stretched. A sub-brow lift is highly effective in these instances.
By making an incision along the lower line of the eyebrow, we:
Remove excess eyelid skin,
Improve sagging of the upper eyelid,
And create a natural eye shape.
The biggest advantage of a sub-brow lift is:

3️⃣ Natural Adhesion Blepharoplasty – Creating a Natural Fold
After sufficiently addressing the eyelid sagging, we created a double eyelid line using the natural adhesion method.

Instead of an exaggerated line, we created a thin, natural double eyelid fold that suits the patient’s age.
4️⃣ Lower Blepharoplasty + Midface Lift – Improving Under-Eye Sagging

Therefore, rather than a simple lower blepharoplasty that only removes fat, we performed a midface lift simultaneously to:

* Post-operative Changes
The most significant change after surgery is the natural improvement in the overall balance of the eye area.
Before surgery, the eyelids were sagging so much that the patient had to use forehead muscles to open her eyes, leading to deep forehead wrinkles. Additionally, the under-eye sagging and fat protrusion created a tired and aged impression.
In this case, we didn’t just perform one procedure; we harmoniously improved the entire structure around the eyes by combining:

Lower Blepharoplasty + Midface Lift
First, through the Endoscopic Forehead Lift, the sagging eyebrows were naturally raised, allowing the patient to open her eyes comfortably without overusing forehead muscles. As a result, forehead wrinkles were significantly softened, and the eyes felt much lighter.
With the Sub-brow Lift, the excess skin on the upper eyelids was trimmed, making the previously heavy eyelids feel lighter and clearing the field of vision. The key was defining the eyes clearly without drastically changing their shape.
Following this, Natural Adhesion Blepharoplasty was used to create a very natural fold. We designed a thin, age-appropriate line to make the eyes look clear without appearing artificial.
The under-eye area also saw a major transformation through the combination of lower blepharoplasty and midface lift. Previously, the protruding fat and sagging skin made the area look uneven and tired, but after surgery, the under-eye area became smooth and flat. Furthermore, the midface lift naturally raised the front cheeks, creating a smooth transition between the under-eye and the cheek area.
As a result:
