About Asymmetrical Eye Correction

Hello!
I am Kim Seong-hoon, a board-certified plastic surgeon and facial designer.
Today, I will be discussing asymmetrical eye correction.
First, let me ask you a question.
Is the client in the following photo asymmetrical?

Accurate diagnosis and evaluation before surgery are extremely important.
For example, the thickness of the skin, the difference in the strength of eye opening, the distribution of fat, the distance between the eyebrow and the eye... etc. After evaluating numerous factors, the surgical method and scope of the surgery are determined.
Although the skin thickness was not thick, there was skin laxity, the distance between the eye and eyebrow was close, there was a small amount of fat, a Mongolian fold was present, and I thought there was little difference in the size of both eyes. Therefore, I performed incisional blepharoplasty, epicanthoplasty, and lateral canthoplasty with the same design on both sides.

The surgical result was my mistake for not recognizing the difference in eye size between the two sides.
The height of both double eyelids is the same, but because the client's right eye does not open well, the right double eyelid appears thicker.
In other words, the cause of the asymmetrical eyes is not that the double eyelid on the right eye is higher, but that the right eye does not open well.
That is, the double eyelid height is not simply determined by how many millimeters it was set to.

Looking at the pre-operative photo again, although it was covered by sagging skin and not visible, there was an asymmetry in the size of both eyes.
As both eyes, which were covered by the double eyelid surgery, were revealed, the asymmetry became more prominent.
Although it was difficult for the client, instead of lowering the height of the right double eyelid, I performed ptosis correction to enlarge the right eye, matching the left and right sides.

Ptosis correction was performed only on the right side, and the double eyelid height is the same on both sides without adjustment.

For asymmetrical eye correction, it is essential to identify the cause of the asymmetry and correct it. If only the superficial, visible results are corrected, it can lead to an unsatisfactory outcome.
This was another opportunity for me to reflect on myself.
I will always strive to be a plastic surgeon who is honest and does not exaggerate.

Today I would like to talk about 'correction of asymmetry of eye'.
Preoperative evaluation is the most important process for accurate plan & range of operation, for example, thickness of skin, fat accumulation, puffiness, height of eye...etc.
A surgeon must correct the reason of asymmetry, not superficial figure. In this case, it was my fault that I didn't recognize the asymmetry of eye height which is covered by skin drooping. After surgery, a covered asymmetry was prominent. I proposed my patient revision surgery for early correction of asymmetry.
The revision surgery was advancement of levator which is the muscle of eye opening, not reducing double fold height of right side.
