At this point, the first treatment option is an implant.

Thanks to daily advancements in dental materials compared to the past,

implants no longer necessarily require thick, long ones to be placed.

Therefore, the surgery period has become very short,

and the prognosis has significantly improved.

Nevertheless,

it takes about 2 to 6 months to complete the implant prosthesis (superstructure),

during which you have to live without teeth.

During this period,

eating can be uncomfortable in the molar area,

and for the front teeth, although there's a temporary denture, it's not aesthetically satisfying.

which many of you encounter these days.

It's not absolutely possible,

and several conditions must be met.

Today, I'd like to explain cases where the prosthesis is placed on the same day as implant surgery.

My front teeth are gapped and loose.

These are frontal and top-down photos.

The teeth are gapped, and the alignment is off.

The diagnosis revealed mobility due to bone loss around the teeth caused by inflammation at the tooth roots.

This is a case where implants will be placed after extraction.

Generally, for anterior implants,

if the fixation between the implant and alveolar bone is not good,

a temporary denture is used

to somewhat address the aesthetic concerns

while waiting for the implant and alveolar bone to integrate.

However,

as mentioned earlier,

there are cases where the prosthesis, which is the superstructure,

can be placed simultaneously with implant surgery.

1. When the fixation between the implant and alveolar bone is good.2. When implants are placed for each tooth (2-3 or more).3. For front teeth, when they don't interfere with the opposing front teeth.

It's not always possible to place the prosthesis simultaneously with implant surgery.

Of course, there might be people who had it done even when the above conditions weren't met and didn't experience problems,

but this is no different from relying on luck with low probability.

The process of creating the tooth crown simultaneously with same-day implant surgery

is called

‘immediate loading’.

The first principle of immediate loading is that the implant must have good fixation with good quality alveolar bone.

No matter how many implants are placed, if the initial fixation is poor,

initial implant fixation failure will occur.

Second, for molars,

assuming the opposing occlusion is stable,

the placement of 2 to 3 implants with continuously good fixation is necessary.

Of course, the force received by the teeth varies depending on chewing force, occlusal force, and habits,

but generally, 3 or more implants are considered stable.

Third, for front teeth, the relatively dense mandibular alveolar bone is more advantageous than the maxillary,

but it may not be possible for patients with very thin bone.

Returning to the case,

this patient's plan is to extract two lower front teeth and place one implant.

Since lower front teeth do not receive much force,

it's possible to place only one implant

and have two tooth crowns.

X-rays showed absorption of alveolar bone around the root

and periodontitis progressing in adjacent teeth, causing mobility.

After checking the CT,

the implant path was meticulously adjusted.

As per the first principle mentioned earlier,

we focused on the fixation between the bone and the implant.

Fortunately, due to good fixation, we used a pre-fabricated temporary abutment and temporary tooth for immediate loading.

In areas where significant bone absorption occurred due to inflammation,

alveolar bone grafting was performed

to maintain gum volume,

and the temporary tooth was completed.

During the period with the temporary tooth, feedback on shape and color

will be reflected in the final prosthesis.

Precautions for Immediate Implant Loading

A special precaution for immediate loading in the anterior region is that the area must absolutely not be stimulated until the implant stabilizes.

After surgery, I ask patients to make a promise.

‘Don't push with your tongue, and don't eat with your front teeth.’

Since immediate loading of front teeth is more about compensating for the aesthetics of a temporary denture than functional chewing,

I strongly emphasize this to patients.

The causes of early implant osseointegration failure are so diverse

that it's important to reduce even the predictable causes.
