Alternatively,

if the anterior teeth are not aligned,

plaque between the teeth cannot be effectively removed,

which can lead to deep cavities.

In such cases, when considering anterior esthetic prosthetics,

let's briefly look at the types available

and how the process unfolds.

The old front tooth crowns did not match in color, and the margins were exposed.

In the past, where the teeth were misaligned,

a cavity formed, and root canal treatment was performed.

To correct the tooth alignment,

this is a case where adjacent teeth also underwent root canal treatment, followed by esthetic prosthetic treatment.

For front teeth, after root canal treatment for a decayed tooth,

rather than just placing a prosthetic on that single tooth,

it is sometimes better to include adjacent teeth in the esthetic prosthetic treatment

to naturally match the alignment and color of the teeth.

Over time, the margin of the prosthetic near the gum line became exposed,

and the color no longer matched the surrounding teeth.

Three prosthetics were removed for remake.

In this case, the prosthetics were made in groups of two and one to enhance retention.

While grouping them together is good for retention,

if a problem arises with a tooth later,

all of them must be removed. Therefore, if retention is good,

it is better to make each tooth individually.

The X-ray also showed misaligned tooth axes

and non-parallel tooth directions.

The teeth were appropriately trimmed to match the receded gum line,

and further subtly trimmed

to improve the insertion angle of the esthetic prosthetics.

This is the day of the final esthetic prosthetic placement.

Compared to before, you can see that the shape and color have become more natural.

If the original tooth color is slightly dark,

additional whitening can yield even better results.

In esthetic prosthetics,

besides crowns, there are laminates.

Recently, they have become very popular, and adhesive technology has advanced,

so they are applied in many cases.

However, laminates do not unconditionally protect teeth,

and 'no-prep' is not always possible.

In cases like the one above, laminates are not indicated.

It would be good to refer to the following case for reference.

Cases where 'no-prep laminate' is commonly possible are

when there are gaps between teeth as shown below,

or when teeth are smaller than normal, known as microdontia.

In other cases, most teeth need to be trimmed to some extent,

and areas with misaligned tooth axes require more reduction.

In cases severe enough to require orthodontics,

whether for laminates or crowns, root canal treatment must precede.

Therefore,

the indications for laminates versus crowns vary by case.

As such, when fabricating anterior esthetic prosthetics,

there are more considerations than for posterior teeth.

All prosthetics, over time,

will have their margins exposed due to gum recession,

so maintenance is important,

and regular check-ups are also crucial.
