About Out-Line Double Eyelid Surgery
Many patients request an 'out-line' double eyelid, but it's often unsuitable for Asian eye characteristics. This article explains why and the conditions required for a natural out-line.

I am Kim Seong-hoon, a plastic surgeon and facial designer.
Today, I'd like to talk about the 'out-line' in double eyelid surgery.
Many people who visit my clinic for double eyelid surgery (hereafter referred to as 'double eyelid surgery') often say, ‘Please give me an out-line’ or ‘Is an out-line possible?’ To give you the conclusion first, the answer is ‘No’ more than 90% of the time. Why is that?
First, let's look at Han Ga-in, an actress many people admire for her beautiful eyes.

Her eyes are truly large and beautiful. Are Han Ga-in's double eyelids an out-line?
Shall we look at photos from different angles?

Strictly speaking, I think they should be described as an ‘in-out line’. That is, depending on the viewing angle (left, right, or from above/below), the front line may appear or be covered. So, which celebrities have an out-line?

If you look at Kim Min-jung, a representative Korean beauty actress, she has typical out-line double eyelids.
Now, back to the main point: how do you get an out-line double eyelid? Simply put, you can create an out-line by using the method of ‘high double eyelid line + removal of epicanthic fold’, but due to the characteristics of East Asian eyes, it often doesn't suit them or tends to roll back into an in-line.

Looking at the photo, the person has not had surgery and appears to have out-line double eyelids, but the epicanthic fold pulls on the out-line double eyelid, creating a multiple fold, which ultimately makes it difficult to call it an out-line double eyelid. In such cases, what if we perform surgery to remove all the epicanthic fold with an epicanthoplasty and create an out-line double eyelid?

Excessive removal of the epicanthic fold can lead to excessive exposure of the lacrimal lake (the red inner flesh exposed during epicanthoplasty), resulting in a fierce or unpleasant eye appearance.
So, should we make the double eyelid higher?

This can result in persistent swelling, no matter how much time passes, making the eyes look like ‘sausage eyes’.
The conditions for an out-line double eyelid in East Asians are:
1. Thin skin
2. Non-puffy eyelids
3. Appropriate distance between the eye and eyebrow
4. Not severe epicanthic fold
5. Lacrimal lake not appearing excessively red
6. Normal strength of the eye-opening muscle
In my clinical experience, only about 5-10% of patients who visit my clinic meet these criteria.
It is always best to find a double eyelid that suits your eyes and proceed with the surgery.

I would like to talk about out-line of double fold surgery.
The most of young asians who visit my office for double fold eyelid surgery, say 'I want out-line, is it possible?'.
The answer is almost 'No, it's impossible' or 'It does not fit on your eyelid'. This is what i want to talk about today. There are few necessary conditions for out-line of double fold.
1) Thins skin
2) No puffiness of eyelid
3) Proper long distance between eyebrow & eyelash
4) Light epicanthal fold (mogolian fold)
5) No prominent lacrimal lake
6) Normal muscle strength of eye opening
