This can cause discomfort in daily life.

For molars, eating might be a bit uncomfortable, but since they are not visible, most people can tolerate it for 2-3 months.

However, for front teeth, living for several months with missing teeth can significantly disrupt social life and cause great inconvenience.

In the past, temporary dentures were provided for this, but as they were removable prosthetics, they often looked unnatural and aesthetically unharmonious.

Recently, if the initial stability of the implant is good on the day of surgery, temporary teeth can be completed on the same day.

Is it possible to have temporary teeth on the same day as implant surgery after extraction due to periodontal disease??

The patient visited us because their lower front gums were frequently swollen.

Two teeth showed bone loss due to periodontitis, leading to tooth migration and gaps.

One of the two teeth had significantly advanced periodontitis, and the adjacent tooth showed mobility of grade 2 or higher, necessitating the extraction of both teeth.

While implant surgery was possible for both tooth positions, considering they were lower front teeth, we decided to place only one implant and connect an artificial tooth to it to complete the treatment, rather than placing two implants.

As mentioned earlier, since treatment can only be completed at least 3 months after implant surgery, we immediately fabricated temporary teeth after the surgery to allow the patient to live comfortably during that period.

Before starting the surgery, we request the fabrication of temporary teeth using an intraoral scanner.

We consult with the dental lab regarding the implant direction and anticipated tooth shape, then modify the fabricated temporary teeth using temporary abutments.

During surgery, for areas with severe periodontitis and significant alveolar bone resorption, alveolar bone grafting is performed to maintain gum volume, preventing gum recession over time.

Temporary teeth were then fabricated using temporary abutments.

This is the appearance immediately after surgery.

The immediate temporary tooth for front teeth implants is not for chewing but for filling the empty space.

Therefore, we adjusted the occlusion with the opposing teeth and modified the temporary tooth to match the gum line.

Of course, the color cannot be perfectly matched like the final prosthesis, but patients tell us it is much more natural than wearing removable temporary dentures.

Time required from surgery to final prosthesis completion

It took about 3 months from surgery to the final prosthesis.

The alveolar bone graft area maintained good volume, and the prosthetic line was naturally crafted.

The color of the final prosthesis was matched to the surrounding teeth, and the tooth shape was designed to fit the gums after the post-surgical swelling subsided.

Since the front teeth are the most visible part when smiling and speaking, several adjustments were made during the healing period to meet the patient's requests.

The patient finally gave their 'okay,' and the final attachment was completed.

The axis of the lower teeth was slightly tilted. While the aesthetic completeness would have been higher if the tooth axis had been corrected and adjacent teeth had also been included in the prosthetics, the patient preferred to only treat the affected area to match their existing teeth, so we aimed for the most natural finish possible.

Although the patient has not been able to visit recently since the treatment was completed, before treatment, subtle mobility was observed in the surrounding teeth. However, after extracting the inflamed tooth and proceeding with gum treatment, the mobility of the surrounding teeth stabilized, eliminating the need for further treatment on other teeth.

Thus, the front teeth play a crucial aesthetic role, making immediate restoration important after extraction.

If the initial implant stability is good, fabricating temporary teeth as much as possible is the ideal treatment method.

While it would be ideal to apply this treatment in all cases, the diagnosis and treatment process can vary depending on the bone quality and surrounding alveolar bone condition.
