Today, I will discuss lateral canthoplasty and lower canthoplasty, which are very popular procedures. Lateral canthoplasty and lower canthoplasty are among the surgeries with many potential side effects, so you must choose carefully.


1. Lower Canthoplasty

You can think of lower canthoplasty as the opposite concept of upper canthoplasty.

While the horizontal or vertical length of the eye does not increase, the area of the white part (conjunctiva) on the outer side (towards the ear) increases, giving the effect of larger eyes.

However, excessive lower canthoplasty can lead to 'scleral show' (where the white part of the eye is visible in three areas: inner, outer, and lower), and it can disrupt the normal flow of tears, causing epiphora (excessive tearing). Therefore, caution is needed.

If we illustrate the normal eye structure and tear flow, tears should flow naturally as shown below. However, excessive lower canthoplasty causes the lower eyelid to sag, making normal tear flow impossible.

2. Lateral Canthoplasty

Lateral canthoplasty is a surgery that can actually extend the horizontal length of the eye.

However, as shown in the diagram, the eye cannot be lengthened horizontally by the full extent of the lateral canthoplasty. This is because the eyeball and face are round.

An important point in lateral canthoplasty is to perform the surgery while preserving the structure of the outer corner of the eye. Otherwise, side effects such as adhesion between the skin and conjunctiva can occur, resulting in an abnormal, 'U'-shaped rounded outer canthus.

If surgery is performed without maintaining the normal ‘<’ shaped structure of the eye corner, various complications can arise.

3. Lateral Canthoplasty + Lower Canthoplasty

Nowadays, various surgical methods are being named and introduced as if they are new procedures, but in many cases, they are a combination of existing techniques. By performing both surgeries together, it is possible to achieve better results and reduce complications that may occur with either lower canthoplasty or lateral canthoplasty alone.

Since the surgery is performed without destroying the angular, pointed structure of the outer corner of the eye, there are no various side effects after lateral canthoplasty as described earlier. The best results are always achieved by performing a surgical method that suits you or is possible for you. You must remember the saying ‘too much is as bad as too little’ (過猶不及), meaning that excessive surgery can actually lead to unfavorable results.

Today I would like to talk about lateral canthoplasty (eye expansion Op in Korea). There are 4 directions for eye expansion: medial, lateral, superior & inferior.

1) Lateral canthoplasty (Lateral expansion Op)

Actual transverse diameter is increased after this Op. The most important point is preserving wedge shaped normal structure of lateral canthus. If this normal structure was destroyed, there will be many complications.

2) Inferior expansion Op

This Op can increase white area of eyeball. Actual eye size or diameter is not changed. This illusion can make eyes look bigger.

3) Lateral + inferior expansion Op

It can fix the Mongolian slant of Asians, so it makes the impression of the face gentle rather than angry.
